Your Breast Augmentation Procedure
Every person’s breast augmentation procedure needs to be different based upon their size, proportions, and desires. Types of Breast Implants
There are two main types of breast implants that our breast surgeons work with: saline implants and silicone implants. Saline implants are FDA-approved for women that are 18 years old or older, while silicone implants are FDA-approved for those 22 and older. Saline implants are made of a silicone gel exterior and filled with sterile saline (salt water). Once they have been inserted into your breast area, they are filled to the cup size that you desire. Saline breast implants have a realistic look and feel, but are more prone to the possibility of rippling or rupturing. However, saline implants require a smaller incision, which can mean less scarring once your healing process is concluded.
Silicone implants are made of a silicone exterior and filled with a cross-linked silicone gel, making them more cohesive. Silicone implants have a higher rate of patient satisfaction because they have a more natural feel and appearance. Silicone implants are also less-likely to rupture, tear, or ripple.
Breast Enlargement with Fat Transfer
Fat transfer to the breasts has become a more reliable procedure over the past few years as the methods of fat preparation have improved. Fat is harvested using regular liposuction techniques from many areas of the body such as the hips, thighs, abdomen, and waist. The fat is then processed so only ideal cells are reused, and then it is carefully injected into the breasts using a micro-cannula technique. The size of the breasts can actually be increased by one full cup size in many instances. This results in a nice improvement while avoiding breast augmentation surgery. Also, because there are no foreign objects being inserted into your body, there is no risk of allergic reaction, which is very rare with breast implants, but still a small possibility. Even though breast implants can give a larger, more predictable result, patients that do not wish to undergo surgery have been happy with the result of fat transfer to the breasts.
Breast Implant Placement Choices
Our plastic surgeons will help you determine the best place for your breast implants to be placed. Typically, breast implants are placed sub-muscularly because it offers the most support and helps hide that fact that you have a breast implant. However, breast implants can also be placed in a sub-glandular position. This is typically only done when a person has atypical anatomy or for women that utilize their pectoral muscles frequently. Female body-builders will often opt for sub-glandular placement over the sub-muscular placement for this exact reason. The final option is called a dual-plane placement where the implant is inserted partially underneath the pectoral muscle wall and partially underneath the breast tissue. This option is meant for women that need fullness added to the upper part of their breasts and allows them to avoid a breast lift procedure.
Breast Augmentation Incision Options
When discussing your breast augmentation procedure with our plastic surgeons, you will also decide upon the best place to make incisions based upon the type of implant you’re getting and your body type. Here are the options you will discuss:
- Inframammary incision – this incision is placed under the breast in the inframammary fold (natural crease beneath the breast.) The implant is inserted through a one-to-one and a half inch long incision. This approach is the most common and scarring is typically unnoticeable and easily hidden by a bra or swimsuit top.
- Areolar incision – the goal of this approach is to hide the incision in the transition from the dark and textured skin of the areola and the lighter smooth skin of the breast. When performed by a skilled surgeon like Dr. Haramis and Dr. Heller, signs of this incision can be difficult to identify.
- Axillary incision – a small incision is created under the muscle in the fold of each armpit. The breast implant is inserted through a channel into a pocket created by the surgeon and heals extremely well.
- TUBA (trans-umbilical breast augmentation) -- a hidden incision concealed under the prominent lip of the belly button leaves no scarring near the breast itself. A tunnel is created beneath the subcutaneous fat layer through which the implant is inserted into the breast cavity.
- Dual plane breast augmentation -- an ideal technique for patients suffering from minor amounts of droopiness and desire a lifted appearance but don’t want to go through a breast lift procedure. During this procedure, a surgeon places the implant partially beneath the muscle and partially beneath the breast tissue. The effect is a lift to the breast along with added fullness, eliminating the need for a breast lift and its required incisions.

Breast Augmentation Procedure and Recovery
Breast enhancement surgeries are performed in one of our accredited surgery centers in New York under closely monitored IV sedation or general anesthesia. You will be discharged and free to return home the same day of your procedure with a bandage and wrap or sports bra in place. Some tenderness, swelling, and bruising are expected. Prescribed muscle relaxants, pain killers, and antibiotics help to make the healing process as comfortable as possible. The breasts will fall nicely into a natural position as time passes. You will need to avoid strenuous activity for two to three weeks, but can typically return to work after 10 days.
Risks Associated with Breast Augmentation
As with any surgery, there are some inherent risks associated. However, the American Society of Plastic Surgeon (ASPS) stated that breast augmentation had become the most popular plastic surgery procedure of 2015 with nearly 280,000 procedures performed. With this level of regularity and experience, the complications that can occur are very rare. These complications are the following:
- Complications with anesthesia
- A decrease or loss in breast sensitivity
- Capsular contracture which means that the breast tissue may harden after the procedure because of the formation of scar tissue

It is hard to put an exact amount on the cost of breast augmentation because it varies so much from patient to patient. The cost will change based upon the type of breast implant, the location of the incision, and the anesthesia charges. Also, if a patient has more risk associated with their procedure than another, this can be an added cost.
