The technique depends on the ear's structure, the size of the concha, and the helical folds. The simplest is a cartilage-sparing technique called percutaneous adjustable closed otoplasty, or PACO.

On the day of surgery, ear reshaping begins with local anesthesia and mild sedation for adults. Children are given general anesthesia.
An incision is made at the back of the ear, near where it attaches to the head. The size depends on how much change is needed and how much cartilage will be removed, introduced, or folded.
The surgeon then creates the necessary bends and folds in the helix of the pinna. The bowl area, which can produce a cup-like shape, is also corrected. This flattens and tucks the pinna. Sometimes non-removable sutures are placed within the tissues to hold the cartilage in position.
These sutures dissolve after a few months and help hold the new shape. After the changes, the incision is dried and stitched closed, then dressed with an anti-inflammatory ointment and a tissue-softening mineral oil. The scar sits at the back of the ear and fades naturally over a few years. The whole process takes about one to four hours depending on the complexity.
Otoplasty has few complications and a quick recovery. Once the anesthesia wears off you may feel some pain, but it's usually mild and comes from swelling. Anti-inflammatory medication can be prescribed if needed.
Keep the dressing on for a day. After it comes off, wear a protective headband for seven days, removing it only to clean the area. After the first week, wear it at night for a month. Clean the area with a warm water compress, then wash with the prescribed medicine. The warm compress also helps relax the cartilage muscles.
Take five days of bed rest after the operation. During this period, avoid washing your head, splashing water on your face, or using a hair dryer.
For children, plan the operation around a one-week rest period afterward. Adults heal within four to seven days and can then return to daily life.
